About the role

As a Care Coordinator, you will support complex care coordination and planning to clients across various funding streams. You will be responsible for developing partnerships with clients to proactively achieve “solution focused” outcomes, including client assessment, completing support plans, rostering care and providing services to meet the client's needs. You will develop positive working relationships within the local service centre and more broadly across the organisation to facilitate a streamlined client experience. Care Coordinators work with Support Workers to promote person centred and reablement focussed care and monitor compliance of service delivery.

This role is a full time twelve month contract and based at our Adelaide office.

What you bring

  • A relevant tertiary qualification and/or experience in the community sector
  • Understanding of and experience working with home care packages
  • Knowledge and understanding of the principles of care coordination, client directed care and reablement
  • Ability to identify risk factors that may affect our clients and assess/develop strategies to support individual care plans
  • Proficiency using MS Office with the ability to learn and adapt to new systems
  • A desire to join a team committed to the care of others in a mission based organisation
