About Us:
Community Team 1 is based at the Royal Melbourne Hospital and holds the homeless portfolio for the Inner West Area Mental Health Service. The team works with consumers living in the CBD and inner city suburbs of Melbourne. Community Team 1 works under an Assertive Community Treatment (ACT) model of care which is characterised by lower caseloads, community based treatment (outreach), team based approach and high intensity of care. The team is committed to working with consumers, their families/carers during their recovery by offering comprehensive and evidence-based (EB) treatment.
Our team is about to enter into a new partnership with Victoria Police to support homelessness in the CDB district, and this role will participate in this initiative.
About You:
-
Experienced, you will present a minimum 2 years' experience working with people experiencing mental illness and demonstrate a strong understanding of mental illness, assessment and risk.
Passionate about working with people presenting mental illness and experiencing homelessness, you are committed to delivering assertive outreach to engage and provide support.
Confident in your ability, you make sound clinical decisions and effectively assess and plan specialist clinical interventions offering support during crises through to recovery planning.
Collaborative, you enjoy working within a multidisciplinary team and with community-based organisations to improve services for the target consumer group.
Dedicated, you hold high professional standards, are accountable for your decisions and committed to further developing your career.
