The Role
The Toowoomba Youth Housing and Reintegration Service (YHaRS) aims to help reduce homelessness through the provision of support, brokerage and accommodation for young people who are; exiting or transitioning from Child Safety orders and/or are transitioning from a period of sentence or remand in Youth Detention Centre’s “sleeping rough”, and living in unstable or temporary housing. The Service is comprised of two components: a support service aimed at young people aged 12-21 years who are homeless or at risk of homelessness to transition to greater stability and independence and an after care service aimed at young people aged 16-21 years who are homeless or at risk of homelessness and are leaving or having recently left the care of Child Safety.
The role will undertake case work and case coordination with young people and assist them to develop and implement case plans which consider their education and training needs, and their health and wellbeing. Working closely with health, housing and other community services this role ensures that the health and welfare needs of young people are met and motivates clients to develop independent living, parenting and social skills.
About You
You will demonstrate strong case management skills including experience in developing and reviewing case plans; have experience working with young people who have complex and multiple needs; knowledge of the issues and barriers that affect young people and knowledge or experience in child protection matters.
Excellent communication and facilitation skills, ability to analyse complex problems and develop solutions and the ability to develop and maintain effective working relationships are vital to your success in this role.
A current Working with Children Blue Card, or the ability to hold one is a mandatory requirement for this role.
